Dr Steiner will present an innovative primary care model for patients with serious mental illness. With the support of NAMI, the local community, and a SAMHSA grant, Wakebrook Primary Care is now in its third year of providing high quality primary care closely integrated with behavioral health care. Wakebrook Primary Care is a UNC Health Care System practice caring for patients with serious mental illness in the Wake community. Dr. Steiner will share components of the model that Wakebrook is using and examine recent data that shows impressive improvements in the quality of primary care for this group of patients. Dr. Steiner will engage the group in a discussion on how to improve this model and how to help spread this model to other communities.
